Who do you treat?
I treat all adults, ages 18 and up.
What services do you not provide?
I do not provide routine gynecologic care. I do not refill prescriptions that are controlled medications (see question 6 below). I do not perform Permanent Disability Evaluations, FMLA form completions, or Workers’ Compensation Evaluations.
Do you take insurance?
I am out of network with all commercial insurances, Medicare, and Medicaid and I operate my practice on a simple “fee for service” model. Payment is collected at the time of each visit. If you wish, I can provide you with an itemized receipt with the necessary information needed for you to submit your own claim to your insurance for possible reimbursement.
Do I have to pay out of pocket for my lab tests or radiology tests?
Laboratory and radiology tests are usually covered and paid for by your health insurance plan. Should you need a radiology test that requires authorization from your insurance, my office can help obtain the authorization so that the test will be approved and covered by your insurance.
Why don’t you take insurance?
Not accepting insurance enables me to provide a higher level of care that most patients desire and appreciate. It allows me to see fewer patients per day, which translates into me being able to spend more time with YOU during each of our visits. Spending more time with you allows me to get to know you better and to address your medical needs in a thorough and timely manner without you feeling rushed. I can be more accessible to you and more easily available for appointments and urgent questions or concerns.
Do you prescribe or refill Controlled Substance Medications?
I do not prescribe medications that have the potential to be either habit forming or misused. Some examples of such medications are: opiates (Oxycontin®, Percocet®) for chronic pain, benzodiazepines (Xanax®, Klonopin®) for anxiety, amphetamines (Adderall®, Ritalin®) for ADHD, or drugs for sleep disorders (Ambien®, Provigil®). To ensure my patients are accurately diagnosed and receive only proper medical therapies, I routinely refer them to the appropriate specialists who have expertise in diagnosing and treating these complex conditions. This policy applies to all patients, including those who are being prescribed these medications by their current or former primary care provider.
How can I provide you with a copy of my medical records?
You may download this Medical Records Form. Send the completed form to your healthcare provider or institution for processing. I prefer all records be sent digitally to my office via our secure fax (not in paper form).
Telehealth?
I currently do not offer telehealth visits to new patients. Once you are an established patient of my practice and we have had an opportunity to get to know one another, I do provide the option of having telehealth appointments for follow-ups or sick visits (when medically appropriate).
What payment options do you accept?
We accept the following forms of payment at the time the service is rendered: cash, check, Square, Amex, Visa, Mastercard, Discover, and HSA debit cards.
